SUMMARY

The Qualified Professional (QP)is responsible for the planning, coordination and oversight of therapeutic and rehabilitative services to consumers enrolled in Enhanced Services. The QP is responsible for implementation and coordination of services in accordance with the PCP; ongoing assessment of the client’s needs; ongoing evaluation of effectiveness of intervention; and, ongoing measurement of client outcomes. The QP provides case management functions, supportive counseling and skill-building interventions.The QP provides clinical oversight and coordination of Associate Professionals and Paraprofessionals in implementing interventions in accordance with the PCP. The QP is also responsible for being a supportive and contributing team member to the client system, families, schools and community agencies. The QP reports directly to the Supervisor of Intensive In-Home (IIH) Services.  All IIH team members will be available to work evening and weekend hours.

DUTIES/RESPONSIBILITIES

•Plan, coordinate and oversee initial and ongoing assessment activities   
•Develop and maintain cooperative and collaborative working relationships with members of the client system. Serve as an FPS Ambassador by positively representing FPS in working with individuals, families and community partners
•Ensure interventions are based on recommendations from assessment activities, are supported by documented medical necessity, and are implemented as documented in the PCP
•Provide ongoing measurement of client outcomes and evaluation of intervention effectiveness
•Mentor and provide case consultation/guidance to Para-Professionals and Associate Professionals to support implementation of interventions as prescribed by the PCP
•Make referrals to recommended services as appropriate and ensure continuity of care as client moves across levels of care
•Develop and implement crisis stabilization plans for clients
•Perform “first responder” duties of crisis response as assigned
•Participates in Intensive In-Home Specific after-hours emergency services on-call rotation
•Provide case management functions such as: linking, referring, arranging and coordinating services
•Identify and intervene to address barriers that impede the development of skills necessary for independent functioning in the community
•Provide information for court reports and other client related reports as required, and attend court as requested and/or needed
•Coordinate discharge planning for clients in hospital, residential services, or other levels of care
•Coordinate and assist clients with community re-entry following hospitalization, residential services, and other levels of care
•Learn and operationalize the program requirements, entrance criteria, continued stay criteria and discharge criteria for utilization of Community Support Services as outlined in Division (MH/DD/SAS) Clinical Coverage Policy
•Secure authorization of services according to established timelines and regulatory requirements as dictated by the Division (MH/DD/SAS), the LME, Value Options and/or other authorizing sources as required
•Maintain client confidentiality in adherence to HIPPA regulations
•Complete in a timely manner, accurate clinical documentation of all services, interventions and client related activities. Maintain this documentation in accordance with standards of FPS file management systems
•Cooperatively and actively participate in all assigned staff and supervisory meetings as well as in service trainings and staff development activities
•Work with supervisor to self-identify training and supervision needs, and ensure services to clients are provided only within the scope of current expertise and abilities
•Consistently maintain established productivity requirements for delivering direct, billable services to assigned clients
•Cooperatively perform other duties as assigned to support the mission and Core Values of FPS

QUALIFICATIONS

•Graduate of an accredited college or university with a Baccalaureate Degree in Social Work or Psychology, or a related human services field and has two (2) years' of MH/DD/SA experience providing direct service to the population served
•All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCAC 27.G.0104
OR
•Graduate of an accredited college or university with a Master’s Degree in Social Work, Psychology, or a related human services field and has one (1) year of MH/DD/SA experience providing direct service to the population served
•All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCAC 27.G.0104
OR
•Graduate of an accredited college or university with a Baccalaureate Degree or a Master’s Degree in a field unrelated to human services and has required number of years of MH/DD/SA experience providing direct service to the population served
•All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCAC 27.G.0104
AND
•All staff providing Intensive In-Home Services to children and families must have a minimum of one (1) year documented experience with this population
