Job Description

Job Purpose: The Liver Services Coordinator will assist in promoting optimal service and care to the patient
referred with liver disease. The coordinator acts as a resource person to referring practices and their staff and
to Liver Disease and Transplant Leadership Team and MDs regarding service and care as related to the liver
disease patient. The coordinator will function as a liaison for liver disease care by maintaining effective lines
of communication with all concerned parties. Supports the mission, vision, values, and strategic goals of
Methodist Health System.

LIVER SERVICES COORDINATOR 
Community Education and Marketing & Physician Outreach Liaison: Acts as liaison
with marketing, transplant center leadership & physicians, and physician outreach
liaison to assure communication (brand) consistency; Distribution of patient and
referring physician education materials; Establishment of a “needs-based dialogue"
with referral sources; Facilitates clinical education for offices and/or facilitates
visit/communication with appropriate physician or clinical team member; Continuously
increases knowledge of disease processes treated within the program in order to
present an educational clinical product to referral sources.

Educational resources and Staff Development: Acts as an educational resource to
promote education of referral sources, inpatient and outpatient staff related to current
concerns, and issues in the referral community with customer service, patient care
issues, or communication.

Liver Disease Program Quality – Inpatient and Outpatient Process Improvement: Utilize
an organized and systematic approach for development of educational, needs-based
dialogue with physician practices based upon pre-planned outreach to practices based
upon identified “status" of practices (i.e. loyal, split-loyalty, loyalty to be developed);
Develops post-visit methodology/reports in order to report back to Leadership and MDs
on clinical questions/issues and/or administrative/customer services questions/issues.

Liver Disease Program Quality – Operational/Administrative Tasks: Actively
participates and/or leads discussions on targeting/status of outreach and customer
service efforts; Prepares monthly visit reports with outcomes and tracks impact on a
quarterly and annual basis; Prepares presentations for strategy and/or operations
meetings.

Patient/Family Liaison: Provides “focused" care coordination in special patient
situations; Coordinates with clinical staff improvements focused on improving the
patient and family experience during liver and transplant care/evaluation.

Job Requirements

Education
Bachelor’s degree or Progressing toward Bachelor’s degree concurrent with, Minimum of 5 years of related
experience in healthcare and sales.
